NOTICE TO CLIENTS – As part of the state’s ongoing efforts to minimize the spread of COVID-19, DUI Counterattack has temporarily suspended all in person operations until further notice. Our phone lines remain open to answer your calls during regular business hours. Out of an abundance of caution, appointments will be conducted with the use of distance learning methodologies to provide continuity of service for our clientele. Registration can be processed online through our website.
DUI Counterattack offers Mail-In-Forms as an alternative to registering online. In order to register by mail you must request either the Mail-In Registration Form and Program Rules by telephone or you may download these forms from this site. Enrollees must reside, work, or attend school in Hillsborough County to register at DUI Counterattack, Hillsborough, Inc., unless an exception has been granted.
After reading the Program Rules and completing the Mail-In Registration Form, you must return both signed forms, along with the items listed below and the appropriate program fee (money order or credit card) to:

The following items will be required prior to the evaluation appointment:

Current picture ID such as State issued driver license or identification card, passport, work ID, VA or military ID, or resident alien card (send a copy - DO NOT SEND ORIGINAL). If you are unable to provide a picture ID, you must have a Social Security card or a birth certificate available.

Official (original) lifetime driving record dated within 30 days of enrollment. Florida driving records can be obtained through our office for a $16.00 fee. Records may also be obtained at the Clerk of the Circuit/County Court or at a local DMV office.

Arrest Affidavit. (Can be obtained at the Clerk of the Circuit/County Court for Hillsborough County arrests.)

Arrest Citation (if available).

Breath test results and/or Blood or Urine Test Results (if applicable).
FAILURE TO PROVIDE THE ABOVE INFORMATION MAY RESULT IN A DELAY OF ENROLLMENT UNTIL PROVIDED.
Upon receipt of the above items along with the signed Program Rules, the Mail-in Registration Form, and the Program Fee, we will contact you at the phone number you provided to schedule your class and evaluation and mail your Class/Evaluation Assignment Receipt.
If you enroll in or attend a DUI Level I Drug and Alcohol Education Course; by giving false or misleading information or by omission or failing to be forthcoming with information concerning previous DUI arrests or convictions or previous attendance at a DUI program, you will be required to re-enrolled and complete the Level II DUI program, including the evaluation appointment. In addition, you will be required to pay the full fee for Level II.
